Below are the requirements for admission:

**Complete Registration Form
*Payment for the desired training course
* Must be a min of 18 yrs old
​
Have or in process of obtaining an AZ Food Handler Card (can be obtained while taking the course)
​
*Food Handlers Card:
All courses will consist of live instruction of all training material though Zoom and Google Classroom
Requirements for Online Live Training:

PC laptop, or tablet
-
Internet access
-
Google classroom access curriculum, tests and other training material (You may need to create a google account if you do not have a gmail account).

​Zoom – access live training (You will need to download the Zoom app which is free)
​
Requirements to work in Arizona Assisted Living Communities:
-
Currently have/in process of obtaining an Az Fingerprint clearance card
-
Currently have/in process of obtaining CPR/1st aid certification
-
Current TB test
-
Possibly a negative COVID-19 test or proof of Vaccine

ADDITIONAL FEES:

Weekend review: $75.00 (scheduled for the 2nd weekend of a current class).

Reprint Certificate: $50.00

Both weekends for Certified Caregiver Course and 2nd weekend includes CNA/LNA/DCW - Bridge Course
CNA/LNA Bridge and DSW Bridge Classes are scheduled on an as needed basis or integrated in the 2ND weekend of CG class.
